Order AP ScoresIf applicable, order scores from your Advanced Placement tests. Use school code 4794 to ensure that the score report is delivered directly to the UCSF School of Pharmacy. These scores must be postmarked on or before the November 3, 2008 deadline. Make PhotocopiesStore photocopies of all of your completed forms and documents in a secure place. For official documents that must remained sealed (e.g., transcripts), we recommend that you order a second copy for yourself. This will assist us if we have questions about your application. Mail Only When CompleteDo not mail your Supplemental Application until you are able to gather all the required materials and submit everything at one time. Review the PharmCAS and Supplemental ApplicationsDo not mail your Supplemental Application until after you have completed your PharmCAS application, reviewed both applications, are satisfied with all aspects of your application, and have submitted your PharmCAS Application. No Additional DocumentsDo not include any additional documents that are not requested in the instructions (e.g., no resumes, curriculum vitae, additional letters of recommendation, etc.) Any such documents will not be included in your file for evaluation and will be destroyed.
