Applications that do not meet the requirements above will be canceled on November 1, 2010.
Applicants who are United States citizens or permanent residents and from low-income backgrounds may apply to have the Supplemental Application Fee waived. To request a fee waiver application, contact our office. If granted, this fee waiver applies only to the Supplemental Application fee of US$60.00. Fee waiver forms from other UCSF schools and other colleges and universities may not be used in place of the UCSF School of Pharmacy fee waiver form.
