Employees urged to submit COVID-19 vaccination records

UCSF is asking all employees working onsite to receive or decline the COVID-19 vaccine by June 30, 2021. Even if you are currently working from home, submitting your vaccination record will help UCSF better understand the rate of vaccination in the UCSF community.

If you received your vaccine at Parnassus or Mission Bay, Occupational Health Services (OHS) has your record and no further action is necessary.​

If you received your vaccine anywhere else, including City College, ZSFGH, or UCSF Fresno, please submit your documentation via the UCSF Occupational Health Portal—you must be on UCSF network or using VPN to access. You can also use the portal to complete and submit a declination.
