Documentation of 1,500 hours of intern experience is required when you file your Application for California Practice Standards and Jurisprudence Examination for Pharmacists (CPJE).
Curriculum (600 Hours):
Our office sends the affidavit for the 600 hours you earn through the curriculum directly to the Board with your application materials. We will send you a copy using the self-addressed envelope you provide when you file for graduation.

Additional Hours (900 hours):
You must provide Pharmacy Intern Hours Affidavits with original signatures for all additional hours. A separate form is required for each site where you earned intern experience.

If your intern hours were obtained outside of California:
You must ask the Board of Pharmacy in the state where your hours were earned to report your hours to the California Board of Pharmacy using Form 17A-16 (License Verification, page 12 of the application).
After gathering your affidavits, submit them with your Application for California Practice Standards and Jurisprudence Examination for Pharmacists (CPJE). Remember, the School of Pharmacy will send the affidavit for the 600 hours you earn in the curriculum directly to the board with your application.
An Affidavit of Intern Experience Obtained in Community and Institutional Pharmacy Settings must be signed and submitted with your application. This affidavit is Form 17A-77 on page 11 of the Application for California Practice Standards and Jurisprudence Examination for Pharmacists (CPJE).
