Deadlines for Expedited Processing

The Board of Pharmacy will expedite the processing of applications for winter and spring graduates who file applications as a group. To be included in this, you must submit your application to the OSACA by the deadline below. The expedited processing is 15 days from receipt; normal processing is 6 weeks.

1. Print, read, and complete all pages.
2. Use the application checklist to prepare submitting materials to the OSACA. Include the checklist when you submit your materials.

3. Ensure your names match. Your name of record with the Board must match identically (letter to letter) with both your government-issued photo identification and federal security card for admission to the CPJE. If they do not, you need to correct your identification so that the names match identically. Original government-issued photo identification and social security cards are required at the CPJE examination site.

   Example: Susan S. Smith and Susan Sarah Smith are not considered identical names.

If you need to change your name, see the instructions on page 2 of Application for California Practice Standards and Jurisprudence Examination for Pharmacists in California (from above).

4. Tape a recent passport photo to Form 17A-1 as directed. If you're in San Francisco, try Passport Depot or City Copy & Print Center and let Web & Data Services Manager Frank Farm (415/502-5372, farmf@pharmacy.ucsf.edu) know if it's good or not.
5. Compile all the materials in the checklist provided.
6. Make photocopies of all of them for your records.

7. Deliver the application checklist and all the application checklist materials to the OSACA. Mailing label:
