Graduation Verification Letters
Our office can issue letters verifying your graduation only after all of your final grades have been submitted. If you need such a letter for an employer, request the letter and ensure that your final grades are reported.
Request the Letter
Send an e-mail to Lucia Piriano (pirianol@pharmacy.ucsf.edu) requesting a graduation verification letter. Specify:
- To whom the letter should be addressed.
- To where it should be sent. Or, if you are in San Francisco, specify "OSACA pick-up" to pick up the letter in the OSACA.
- The state for which you are applying for licensure (e.g., California).

Ensure Final Grades Are Reported
- Complete all of your graduation requirements -- including submission of final projects and reports.
- Pharmaceutical Care Students: Offer to hand carry the final APPE evaluation back to your program director so that your grade can be recorded. Delays in receiving the APPE evaluation from the preceptor often limits Clinical Pharmacy's ability to assign grades.
