Filing for Graduation

To file for graduation: Complete the 3 forms below and send them to our office so that we receive them on or before your filing deadline. These forms require Adobe Reader.

If the filing deadline falls on a weekend or University holiday, the deadline is automatically moved to the following business day.

Send us 3 forms, 1 check, and 1 self-addressed envelope:

1. Intent to Graduate Form

2. Pathway Worksheet
   Use only the worksheet for your pathway:
   • Pharmaceutical Care Worksheet

   • Pharmaceutical Sciences Worksheet

3. Transcript Request Form

4. A US$10.00 check for the Transcript Request Fee made payable to UC Regents.
5. A self-addressed envelope (standard business size #10: 9.5" x 4.125"). We will mail a copy of the affidavit for the 600 hours you earn through the curriculum to you at this address.

   Since your copies will not be sent until after you graduate, please use a permanent address!
