    • Check Mailing Information and Authorization (Clear all holds and file your intent to register at least 2 weeks before checks are mailed for the following quarter)

    • Direct Deposit Request (For UCSF funds only)
    • Educational Expense Request (For uninsured medical/dental expenses, car loans, computer expenses, etc.)
    • Entrance Interview Questionaire
    • Financial Aid Update Form (For providing additional information and declining loans)
    • Income Tax Declaration Form (For students and/or parents that did not sign their tax returns)
