Complete the Required Background Check

UCSF requires a background check in order to:

  • verify an individual’s suitability to participate in experiential education rotations
  • confirm a student’s eligibility for pharmacy licensure, and
  • ensure patient safety.

UCSF participates in the PharmCAS-facilitated national background check service for all students who are offered provisional admission. All provisionally admitted students are required to submit to a national background report at the point of initial, conditional acceptance. UCSF will be notified if consent is not provided. Failure to provide consent will result in failing to meet our admissions requirements. Upon accepting an offer of admission, you will receive an email from [email protected] with the subject line “Background Check Consent Request.” Please follow the specific instructions in the email.

If you've already completed a background check through Certiphi for another school, you will not receive an e-mail from Certiphi. No further action on your part is necessary as Certiphi will automatically send us your completed report.

UCSF reserves the right to rescind an offer of admission to any individual whose background check reveals unprofessional conduct inconsistent with the high standards of the pharmacy profession or issues that may preclude an individual from participating in Introductory Pharmacy Practice Experiences (IPPEs) or Advanced Pharmacy Practice Experiences (APPEs). Under these circumstances, the Admissions Committee will review the application and provide a recommendation to the Vice Dean for Education. The Vice Dean for Education may revoke an offer of admission based upon this review. The decision of the Vice Dean is final and is not subject to appeal.
