Complete the required background check
UCSF requires a background check in order to:
- verify an individual's suitability to participate in experiential education rotations
- confirm a student's eligibility for pharmacy licensure, and
- ensure patient safety.
UCSF participates in the PharmCAS-facilitated national background check service for all students who are offered provisional admission. All provisionally admitted students are required to submit to a national background report at the point of initial, conditional acceptance. UCSF will be notified if consent is not provided. Failure to provide consent will result in failing to meet our admissions requirements. Upon accepting an offer of admission, you will receive an email from studentedition@certiphi.com with the subject line "Background Check Consent Request". Please follow the specific instructions in the email. Reports will be reviewed prior to final admission offers being made. To increase the likelihood that this important message is not accidentally treated as spam, please add studentedition@certiphi.com to your address book.
