submitting transcripts for the UCSF supplemental application

As part of your supplemental application, you must submit official copies of transcripts from:

  1. Canadian institutions where English is not the language of record,
  2. Colleges and universities located outside the United States and Canada, and
  3. High schools. You must submit an official copy of your high school transcripts if so indicated by question E6 on Form E: Application Agreement.

Submit Official, Sealed Documents Only

Our office must receive official copies of transcripts for all college level course work directly from the issuing institution.

Other Colleges and Universities

Send transcripts from schools that don't fit into the categories above to PharmCAS instead. See Submitting Transcripts for the PharmCAS Application.

Foreign Institutions

If applicable for your country, you must also submit an official copy of your degree certificate. For additional details about document requirements from foreign institutions, see all of the following:
