On Form C, how do I list lecture and lab grades?

You must only list the lecture and lab grades separately if each is assigned its own course name and number and listed separately on your transcript.

List the grade you received for each course.

If it won't fit on one line, abbreviate, use 2 separate lines, or use a second copy of Form C.

Only official grades listed on your transcript should be reported on Form C.
