On Form C, how do I list lecture and lab grades?

You must only list the lecture and lab grades separately if each is assigned its own course name & number and listed separately on your transcript.

List the grade you received for each course.

If it won't fit on one line, abbreviate, use 2 separate lines, or use a second copy of Form C.

Only official grades listed on your transcript should be reported on Form C.
