Fall 2009 Supplemental Application Now Available
Friday, July 18, 2008 - The UCSF Supplemental Application for students entering in fall 2009 is now available. If you are applying for entry in fall 2009 and had downloaded any supplemental application before 2:35 PM on Friday, July 18, 2008 you must download and use these forms instead. Another way to check: Old forms say Fall 2008 at the top; new (current) forms say Fall 2009.

Instructions for Completing Form D

You must complete this form on a computer using Adobe Reader or Adobe Acrobat. If you're not familiar with these software products, see About Adobe Reader.

Download and Save Before Starting
We recommend that you download and save the forms to your computer and complete them in Adobe Acrobat or Adobe Reader. If you instead view and complete the forms in your web browser, you might lose data you entered if you select a hyperlink which takes you to a different web page. Saving the forms to your computer and completing them in Adobe Acrobat or Adobe Reader prevents this problem.
Saving the Information You Enter
If you use the free Adobe Reader, be aware that Adobe Reader does not permit you to save the information you enter like Adobe Acrobat does. For details, see About Adobe Reader. If you don't have Adobe Acrobat and you don't want to purchase it, we recommend that you:
- Check what other resources available to you might have Adobe Acrobat: your public library, the computer lab at your school, nearby Internet cafés, your friends or family who are knowledgeable about computers. For example, work at Kinko's, saving your forms to your own USB drive.
- Or, complete each form only when you can do so in a single sitting where you have the ability to print. (You do not need to complete all the forms in a single sitting.)
Adobe Acrobat is not required to complete our forms, and UCSF will not reimburse you for purchasing it.

It is your responsibility to be aware of all policies concerning prerequisites as outlined in Academic Prerequisites. All prerequisites must be completed by September 1, 2009.

You must account for all prerequisites with completed, in progress and/or planned course work that has been approved by the Office of Student & Curricular Affairs of the UCSF School of Pharmacy. Petitions for course substitutions are not accepted. See Verifying Your Prerequisite Course Work to determine what courses from your school are approved to meet the prerequisites.

The document does not allow you to add or delete rows. If more space is required to completely list your prerequisite coursework, use a second copy of the form. Please do not use other lists or forms to provide this information.

List all units as quarter units. One semester unit is equivalent to 1.5 quarter units. To convert semester units to quarter units multiply the semester units by 1.5.

If the data you enter for a cell doesn't fit on one line, abbreviate it. For example, instead of "Advanced Organic Chemistry" use "Adv Org Chem."
Withdrawn/Repeated Courses
If you withdrew from or repeated a prerequisite course, all instances of the course should be listed in chronological order. You must provide all of the information requested for each time you were enrolled in the course. If you do not list repeated courses, your application will be canceled.
In Progress/Planned Coursework
- Specify "IP" in the grade box for course work in progress during the fall 2008 term.
- Prerequisite course work planned for winter, spring, or summer 2009 must also be listed. You should provide the information requested for all fields except grade (enter "Planned").
AP Test Scores
If you use AP scores to fulfill any prerequisites, you must list the test name, test score, and year taken. Specify the score earned in the grade field and units awarded in the units field. You may leave the school field blank. See Advanced Placement Tests to determine if your AP test scores qualify to be applied towards prerequisites.
