For all steps in the application process, it is your responsibility to follow all instructions and meet all deadlines. Failure to do so might result in your application being canceled.
The application consists of 2 parts. Both parts are required.
To be eligible to apply, applicants must have a minimum, cumulative undergraduate GPA of 2.80 as calculated by PharmCAS as of the November 1 deadline. For information about GPA ranges of admitted students, see About the Students and Applicants.
Pharmacy College Application Service (PharmCAS) Application
PharmCAS is a service that enables you to use one primary application to apply to multiple PharmD degree programs. You fill out these forms on the Web, then submit them electronically.
Supplemental Application
Information that PharmCAS does not collect, but that is required by UCSF to complete our comprehensive process of evaluation.
You download and print these forms, then submit them through regular post (e.g., U.S. Postal Service).
We will offer a limited number of Supplemental Application Workshops that are designed to help applicants complete these materials correctly. Dates and registration information will be posted later this summer.
Deadline: November 1, 2008
Late applications are not accepted under any circumstances.
You are required to submit official copies of your transcripts (also known as "marks reports"). Some transcripts must be sent to PharmCAS, and some must be sent to UCSF. For details, see Checklist: Transcripts.
AP test scores may be used in lieu of course work to fulfill some prerequisites, but no other placement exams are accepted. You cannot apply the IB, CLEP, or in-house placement tests offered by your college or university. For details, see Advanced Placement Tests.
