Change of Address, Telephone, or E-mail
If you are an applicant, you must notify us of any changes in your contact information:
- current mailing address
- permanent mailing address
- telephone numbers
- e-mail address
by reporting the changes to the Office of Student & Curricular Affairs.
How to Notify Us
We require a written notice. Please include:
- Your full legal name

Your PharmCAS ID number
- The date the change is effective.
